Central Health Medical Access Program (MAP) is now scheduling in-person appointments for individuals applying for MAP.  Call 512-978-8130 to schedule an appointment at one of the following locations – Central Health Southeast Health & Wellness Center located at 2901 Montopolis Dr. Austin, Tx 78741 Monday – Friday 8am-5pm or the Central Health Northeast Health Resource Center located at 6711 Johnny Morris Rd Austin, Tx 78724 Monday – Friday 8am-5pm. Individuals experiencing homelessness may be seen on a walk-in basis at Southeast Health and Wellness, all other applicants can continue to apply online at www.centralhealth.net/get-started or over the phone by calling 512-978-8130
