National nonprofit Communities Joined in Action (CJA) announced Sept. 10 that Central Health is being awarded the organization’s 2015 Healthcare Transformation Award.
CJA is comprised of nearly 150 community health collaboratives from throughout the U.S. Each year, CJA’s members select an individual, agency or coalition for recognition in developing a “replicable and transferable idea, tool, project, or learning that has value in community safety net care.”
Central Health was chosen for the 2015 Healthcare Transformation award based on its work:
- Leading a central learning collaborative designed to coordinate best practices in patient engagement among Central Texas 1115 Medicaid Waiver providers;
- Developing an integrated delivery system for improved patient health care through the Community Care Collaborative; and
- Spearheading initiatives to increase education and enrollment in Affordable Care Act health insurance plans.
Central Health will be recognized for its work Oct. 1 at CJA’s annual conference in Washington D.C. Central Health will also produce a webinar highlighting its work to serve as a guide for safety net health care providers throughout the country.
Central Health was nominated for the award by the Texas Health Institute, a nonprofit, nonpartisan think tank dedicated to improving health care in Texas through partnerships in the public and private sectors.
