Central Health, in its role as the Anchor entity for Regional Healthcare Partnership (RHP) 7 is producing a series of videos as part of the public stakeholder engagement process described in the Texas Healthcare Transformation and Quality Improvement Program – Medicaid 1115 Waiver Program.
Community stakeholders in Travis, Hays, Bastrop, Caldwell, Fayette and Lee Counties asked both Central Health and health care providers participating in the Waiver for more information about the local impact of Delivery System Reform Incentive Payment (DSRIP) projects being performed in their communities. Central Health chose to respond to these requests by creating videos, which will demonstrate the importance of these programs in the lives of the people they serve and the overall impact on the health care system.
The complexity of the 1115 Waiver lies in its scale and performance protocols. RHP 7 is home to 77 DSRIP projects operated by 10 diverse health care partners throughout the six-county region.
“Public Health Departments, public and private hospitals, and mental health authorities are working in six different counties to expand primary care capacity, increase specialty care, extend behavioral health services, and invest in public health. These efforts all lead toward achieving the Triple Aim – better quality individual care and improved population health outcomes at a lower cost,” Central Health and Community Care Collaborative Medicaid Waiver Director Sarah Cook said.
The first video in this ongoing series, “Servicios Médicos Móviles,” highlights the Community Care Collaborative’s (CCC) Mobile Care Teams project that is being implemented by Central Health’s affiliated Federally Qualified Health Center system CommUnityCare in locations throughout Travis County. The remaining seven videos will be released in the coming weeks.
“CommUnityCare is proud to serve people who might not have access to a family doctor with our dedicated mobile teams. Many of the locations where the mobile teams set up are in medically underserved areas that do not have a health clinic nearby. This project is helping us fulfill our mission to provide the right care, in the right time, at the right place,? CommUnityCare CEO George Miller said.
Beginning on Monday, Jan. 26, patients will now be able to either drop into a mobile clinic location for a primary care appointment or schedule an appointment at one of these locations through the patient navigation center at CommUnityCare by calling 512-978-9015. Within coming months, there will be a new mobile care team focused on street medicine providing care to individuals experiencing homelessness across Travis County.
